DESARROLLO DEL PROTOCOLO DE CONCILIACIÓN DE MEDICAMENTOS PARA PACIENTES ONCOLÓGICOS

Descripción:The implementation of a protocol for oncology patients, as well as medication reconciliation in a hospital, is a tool that improves user safety through the detection of medication errors and more comprehensive patient care. General objective: Contribute to the reduction of medication errors in the Cancer and Hematology Center of the Metropolitan Hospital by developing a medication reconciliation protocol that improves the safety of patients who begin treatment at this Center. Methodology: review and development of a medication reconciliation protocol composed of five sections, which allows the detection of errors and discrepancies in oncology patients at the medical center. Results: A simple, clear, and efficient protocol was carried out. With the application, a total of seven justified discrepancies were found and three interventions could be identified. Among the discrepancies, two of them were grade X interactions, three were severity D, and two were serious and moderately serious medication omissions. The interventions carried out are considered two as serious and one as moderately serious. Conclusions: Although medication errors as such were not detected, important discrepancies were discovered to be clarified, and interventions allowed for improving patient safety.
